Please bring your medical insurance cards, photo ID, and completed forms into your appointment, along with any required insurance referrals.
In addition for all appointments, bring current eyeglasses and contact lens boxes.
For all appointments, please bring current eyeglasses and contact lens boxes or contact lens specifications, a list of current medications and your insurance information and photo ID. Please allow 1½ to 2 hours for complete eye exams. Please be aware that the order of patients being called back for their appointment may vary with certain factors, including which doctor they have an appointment with and what type of appointment they have. When scheduling appointments, please also be aware that your eyes may be dilated and hence sun-sensitive after the appointment. For dilated examinations, it is advisable that you bring sunglasses. If driving after dilated exams has been a problem in the past, we encourage you to bring a driver for your return from these appointments.
Your visit will occur in several stages. Initially, a skilled technician will perform preliminary testing. Some conditions require specialized testing as well which will often be performed prior to your seeing your doctor. Then at your examination with your doctor, you will be examined and any testing that was done will be interpreted for you.
Our doctors are Medicare-participating providers and we participate in most major insurance plans (but not Medicaid or other government-funded plans):
Please cancel/reschedule your visit with at least 24 hours’ notice. A $50 no show fee will be assessed without 24 hours’ notice; same day reschedule is also considered a no show. No show fees must be paid before you are allowed to schedule another appointment. We try to provide a computerized reminder call 2 days prior to your appointment; however, reminder calls are a courtesy and it is your responsibility to remember any appointments you have scheduled.
